New Patient Registration
If you live within our practice area, you are welcome to register with us and our reception staff will be happy to help you with this form. We can quickly tell if you are within our practice area from your address, so please bring proof of this and bring a recent gas, electricity or rates bill and also one photographic ID such as passport or driving licence.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to come from your previous doctor.
After you have registered, new patients can make an appointment to see the healthcare assistant for a free health check. We can take details of your previous medical history and any other problems that we can help with. It also gives you the chance to find out about the services we offer.
